In 1958, the American Medical Association (AMA) published “A Guide to the Evaluation of Permanent Impairment of the Extremities and Back”3 in the Journal of the American Medical Association; this was followed by additional guides published in that journal. The First Edition was published in 1971 as a collection of 13 manuals.

The Second Edition5 was published in 1984, and the Third Edition6 was published in 1988. Third Edition was the first to use the Swanson methodology7, which assigned specific impairment ratings to upper-extremity range of motion (ROM) deficiencies.
